Possibilities of influencing compliance with following diabetic regimes from the perspective of nutrition therapist

Diabetes mellitus is a metabolic disease that occurs either when the pancreas does not produce enough insulin or when the body cannot effectively use the insulin it produces. Only in the Czech Republic, we detect over one million people diagnosed with this disease. By 2045, over 700 million people are predicted to be diagnosed with diabetes worldwide (Global diabetes data report 2000 - 2045 (diabetesatlas.org)). Medical community and pharmaceutical companies give big importance and pay significant attention to the development of innovative ways of treatment. Nowadays, there is a wide range of treatment available, but only hand in hand with the right compliance and patient's motivation can medical treatment bring the desired outcome. This bachelor thesis aims to explore the level of awareness among diabetic patients about the disease itself and nutrition knowledge through out a questionnaire survey. The main part of the thesis describes factors that can help with the patient's motivation. Questionnaire survey was used in a set of 271 patients treated in Klatovska nemocnice. Results of the survey are presented in the form of graphs. Results of the survey show that men are more satisfied with the ongoing threatment than females. Contemporary nutritional recommendations and their compliance by nutritional therapists

This work is focused on Czech and world nutritional recommendations and the profession of nutritional therapist. The aim of the work is to analyze the eating habits of nutrition therapists as nutrition experts. The work is divided into chapters, within which a comparative analysis of both nutritional recommendations and all levels of the profession of nutritional therapist is performed. The analysis of identical and different points was performed on the basis of the study of documents and their comparisons. The first part of the thesis deals with the issue of nutritional recommendations, their importance, forms and methods of creation. The following is a list of current recommendations valid in Europe and in the Czech Republic. These recommendations are then compared. Furthermore, attention is paid to the profession of nutritional therapist, its legislative anchoring and competencies. As this profession has several levels, they are compared with each other and the differences are emphasized. Subsequently, the individual activities of a nutritional therapist are explained and practical examples are presented. Last but not least, an analysis of the eating habits of the addressed nutritional therapists is prepared and compared with the current nutritional recommendations valid in the Czech Republic....
THE USE OF ASSESMENT OF HUMAN BODY CONSTITUTION AND ENERGETIC NEED FOR NUTRITIONAL THERAPIST

The thesis entitled "The use of assesment of human body constitution and energetic need for nutritional therapist" is aiming to clearly and briefly introduce respectiv methods and assess their positives and negatives in common nutritionist's practise. The comparison of each methods is not only about the results, but mainly about the use for nutritionists. Theoretical part of the thesis consists of chapter describing human energy requirements and their asessment methods and chapter about the human body composition and its assessment methods. I atemt to collect the latest information within simple structure. This part is focused mainly on methods which are used in practical part of the thesis. It is not dealing with complicated inaccessible methods, which use is limited to academic purposes, in detail. The practical part of the thesis uses qualitative research method. The corpus consist of 7 respondents. The data are collected in form of questionnare followed by notes from each measurment. The further view expands interview with respondents, whose komfort was examined during different measurments. The assessment of primary human energety requirements is done by using: indirect calorimetry (QuarkRMR), bioimpedance (InBody230, Bodystat 1500), and the calculation using Harris and Benedict equation. The assessment of human body composition is done by using: bioimpedance (InBody230, Bodystat 1500), the conversion tables by Hronek (2013) using Caliper which measures skin kelp on the top of the hip bone, Deurenberg's equation and the use of tape measure for measurment of waist circumference. I am focusing on composition of body fat mass in percentages, because it mirrors the condition of an individual well. (Kutáč, 2009, Stránský and Ryšavá, 2014) The final analysis is done by Excel. There, I am assessing the results of each respondent and to comparing the resoluts of different methods among each other. The work is supplemented by graphs and tables, which ease the comparation. At the end I am evaluating respondent's on all methods and interests in the results. If the possible expectations were fulfilled. In the final part of the thesis I am assesing the methods and thein results for nutritionist's daily practise. The biggest diference in assessment of primary human energy requirements is 427kcal. Despite of that I consider all used methods as useful for nutritionist. The exeption is the most important index is body weight development, which we have to observe regularly. At the end of each chapter I assess the methods and their results for nutritionist's work. The biggest different in assesment of basic energetic need is 472 kcal. In spite of that I consider all of the methods as usefull for nutritionists. The most important is body weight development, which we have to observe reguraly. In assessment of human body composition more precisely the composition of body fat mass I found big differences in results. The biggest one is 15%. This might be caused by bad preparation stage of respondent before the measurment. Despite the big differences I consider all methods, that were used good for nutritionist. Mostly to see the development of the body composition, which can be monitored uses one method, in the long term period.
Nurse's role in the nutrition of patients in intensive care

This bachelor thesis is divided into two parts theoretical and empirical and the purpose of this thesis is to deal with the topic of the nurse´s role in the nutrition of patients in intensive care. In the theoretical part of the thesis, complex issue of the nutrition ensure of intensive care patients is summarized. We have been dealing not only with the common concept of the nutrition on the intensive care units and the role of the nutrition therapist but also with the characteristics, causes and diagnosis of the malnutrition as the most serious issue in the nutrition field. In the thesis, the types of a nutrition support in the intensive care have been also defined as well as the indication, contraindication, types of serving, complications, types of medicines, monitoring and the related role of the nurses. The practical part provides with the results of the research, which have been found by using a qualitative research method. The main purpose of this bachelor thesis was to emphasize the role of the nurse by the handing enteral and parenteral nutrition and also a description of the cooperation with the nutritional therapist on the associated intern and neurological unit of intensive care at České Budějovice hospital. At the basis of above mentioned, two objectives have been stated. At first, our task is to find out the way how is the right handing of the nutrition to the intensive care patient by the nurse secured. The second task is finding out the way of cooperation of the nurse and the patient while securing the nutrition of the patients in the intensive care unit. For the first task, four research questions have been established, for the second task have been established one research question. For the high-quality data network, the method of hidden monitoring for keeping authenticity has been used. As a consequence of the insufficient amount of data to the reaching of the second task, half-structured interview has been chosen instead. Research dataset consisted of thirteen records of sighting and one interview on the above mentioned intensive care unit. As a basis for analyses, three observation sheets were made, into these sheets were then during the observation information assigned. Each sheet consisted of single points planned in accordance to theoretical part. The nurse follows these points when securing the patient´s nutrition. Remaining information, not included in observation sheet were written into the notes section or were included into the information about the performing of the particular point. The results were analyzed by using an open-coding. There are six categories coming out from the analyses, which are connected with securing the education, information providing, keeping an obstacle-free approach complications preventing, using tools, nurse´s cooperation with the patient and deciding processes during work performance. In the results, there is a conclusion of the analysis with illustrative examples to find. The sequence of the results is presented according to sequence of categories during performing. In each category, the results are evaluated according to research tasks stated. We believe, that the obtained information, which map the activities of the nurses when securing the nutrition at the intensive care unit patients and the acquainting the cooperation with the nutrition therapist helps in the future to the awareness of the potential weaknesses and following correction. The results may become inspiration for more detailed analysis of this topic.
